Purpose: This document outlines the standardised reporting format to be used by all empaneled NQAS Assessors who have conducted assessments at either the facility level or district level in Mizoram. This standardised reporting format ensures consistency and clarity in the communication of NQAS assessment results at both the facility and district levels.

• District-Level Assessments: Assessment reports for the district level must be submitted to the District Quality Assurance Committee (DQAC) for review in consideration of applying for the NQAS State Certification Assessment.
• Facility-Level Assessments: Assessment reports for the facility level serve to summarise the findings for discussion during meetings of the Rogi Kalyan Samiti (RKS), Jan Aarogya Samiti (JAS), and/or Quality Team meetings.
